  • Draft the affidavit in a standard affidavit format. You can get the format easily online. You just need to include your personal details, the reason for the name change, old name, new name after marriage, husband's name, the date of marriage, and so on.

  • Print on Non-Judicial stamp paper of the required value.

  • Your two witnesses and you sign the affidavit in the presence of a notary public or an advocate.

  • You can also publish an advertisement about the name change in two local newspapers (one in English and one in the regional language).

  • Optionally, you can also publish an advertisement in a national newspaper. 

  • Submit the affidavit and the newspaper cuttings to the office of the Registrar of Births and Deaths or the Gazette Office for publication in the official gazette.
