What are the documents required for family pension?

Documents needed from Family Pension Applicant:

  • A letter of request for a family pension

  • Request for Family Pension Application (in Duplicate)

  • Affidavit with proper Executive Magistrate/Competent Authority stamp and attestation

  • The authentic death certificate

  • ID proof with spouse's name and date of birth

  • Three (3) photos the size of a passport

  • A copy of a Pension Payment Order (PPO)

  • Copy of a bank account (SBI or PSB)

Documents required for family pension in cases where the deceased pensioner and spouse were holding a joint account: 

  • A straightforward correspondence or application form to start a family pension.

  • Pensioner's death certificate, in PDF format. a copy of the pensioner's PPO, if any.

  • Evidence of the applicant's age or birthdate.

  • The spouse or family member is not required to give Form 14 to the bank in order to start the family pension.

  • The Form 14 proposal must have the signatures of two witnesses if the spouse did not have an account with the dead pensioner.

  • Pensioner's death certificate, in PDF format.

  • A copy of the pensioner's PPO, if any.

  • Evidence of the applicant's age or birthdate.

  • Form 14 does not require to be validated by a Gazetted Officer, etc. The issuing bank will identify the spouse or family member using the details in the PPO and it's own "Know Your Customer" criteria.
