Documents Required:Birth Certificate/ School
Certificate, SFC certificate, to verify the age.
Proof of address like ration card, driving license, Aadhar card, water bill, telephone bill.
Residential proof like residence proof by Talathi, residence proof by Gram Sevak, Residence Proof by Bill Collector.
Self Declaration Form (Mandatory).
Filled Application form
Identity proof like Voter ID, Aadhar Card, passport, driving license.
Xerox copies of all the documents attested by a government official.
An affidavit from the court.
To get the application form you can opt for online means or you can go to Sub-Divisional Magistrate's Office, Tehsildar's Office, or District Magistrate Office if you are applying in the offline mode.
